Effects of zinc supplementation in liver cirrhosisLiver cirrhosis can lead to serious long-term complications. Common causes include nutrient deficiencies, excessive alcohol consumption, and chronic liver inflammation. Many people also have the precursor condition known as non-alcoholic fatty liver disease (NAFLD), often without being aware of it.
A meta-analysis published in BMC Nutrition shows that zinc supplementation improves liver function across several parameters in patients with cirrhosis.

Cirrhosis is a progressive disease in which healthy liver tissue is gradually replaced by scar tissue (fibrosis) following prolonged damage. In the Western world, the most common causes are heavy alcohol consumption and non-alcoholic fatty liver disease, which is often associated with overweight, type 2 diabetes, and metabolic syndrome. Chronic hepatitis, autoimmune diseases, and genetic factors can also increase the risk.
Overall, cirrhosis is characterized by persistent chronic inflammation and oxidative stress, in which free radicals attack healthy tissue, leading to its replacement with scar tissue. This results in chronic liver failure with symptoms such as fatigue, nausea, weight loss, jaundice (yellowing of the eyes and skin), as well as localized fluid retention, pain, and shortness of breath.
In addition, cirrhosis may lead to hepatic encephalopathy (“brain toxicity”), which can cause sleep disturbances, cognitive decline, or even coma.
Although cirrhosis is considered a chronic condition, treatment aims to eliminate the underlying causes, such as excessive alcohol intake or unhealthy dietary habits. Zinc deficiency is also known to be widespread among patients with cirrhosis, but the use of zinc supplements has been somewhat controversial. This prompted the researchers behind the new meta-analysis to examine zinc’s role more closely and to study its impact on the clinical management of cirrhosis and hepatic encephalopathy.

Zinc supplementation improves liver function across multiple parameters

Using databases such as MEDLINE, Embase, Cochrane Library, Scopus, Ovid, and Web of Science, the researchers identified relevant studies published up to March 2025. They found seven studies including a total of 338 patients with cirrhosis, with 172 receiving zinc supplementation and 166 serving as controls.
The meta-analysis revealed that zinc had the following beneficial effects:

  • Increased serum albumin, a protein produced by the liver that helps regulate fluid balance
  • Reduced levels of alanine aminotransferase (ALT), a marker of liver damage
  • Lower levels of toxic ammonia in the blood, which the liver normally converts into urea
  • The most pronounced benefits were observed with daily zinc doses below 50 mg administered for 16 weeks or longer

The researchers therefore conclude that zinc supplementation in patients with cirrhosis may lead to moderate improvements in nutritional status and liver function, thereby reducing the risk of hepatic encephalopathy. However, further studies are needed to determine optimal dosing and treatment duration.

Zinc supports liver function in different ways

Zinc is an essential trace mineral involved in hundreds of enzymatic processes that are vital for overall health and for activating a wide range of genes, including those involved in liver metabolism.
It also plays an important role in immune function, regulation of inflammation, and as a key antioxidant via superoxide dismutase (SOD), protecting liver cells and other tissues from damage caused by free radicals and oxidative stress.

Widespread zinc deficiency and the vicious cycle in cirrhosis

Zinc is most readily absorbed from animal sources such as meat, fish, eggs, and dairy products. As a result, plant-based or unbalanced diets may increase the risk of zinc deficiency. Other contributing factors include low stomach acid, alcohol abuse, certain intestinal disorders, and the use of diuretics, all of which can impair zinc absorption or increase its excretion.
Inherited zinc deficiency is most commonly associated with mutations in a specific gene (SLC39A4), which affects zinc absorption and utilization.
Liver diseases themselves may also contribute to zinc deficiency. This creates a vicious cycle, as low zinc levels can further worsen the progression of liver disorders, including cirrhosis.

Zinc requirements and supplementation

Official recommendations for zinc intake are typically around 10 mg per day. However, according to this meta-analysis, patients with cirrhosis appear to have higher requirements and may benefit from daily supplementation of up to 50 mg. This is the case even though the European Food Safety Authority (EFSA) has set an upper intake level of 25 mg per day.
Because cirrhosis is a chronic condition, supplementation for longer than 16 weeks may also be beneficial.

  • An estimated 46% of the global population is affected by zinc deficiency
  • Zinc deficiency is particularly common among patients with liver diseases, including cirrhosis
  • At the same time, zinc requirements increase in cirrhosis and other liver conditions

Additional recommendations for the prevention and management of cirrhosis

Since unhealthy diets, multiple nutrient deficiencies, non-alcoholic fatty liver disease, and alcohol abuse are among the most common causes of cirrhosis, improving dietary and lifestyle habits is essential for both prevention and treatment.
